Directions:
1. Print the required form.
Complete the form with all of the required information.
Enclose the required processing fee (note only cashiers check or cash accepted).

2. Mail the form to:
FVSM.

3. Once received, it will be completed & mailed.
This process takes any where from 2-4 weeks once it is received by FVSM.

Processing will take 2 - 4 weeks from the date received.
During the months of May, June, July, August, September requests may take up to 8 weeks.
